(Irlanda), el Dr. Jubil Thomas, especialista en anestesia, demostr\u00f3 la capacidad de la oxigenoterapia de alto flujo (<a href=\"https:\/\/www.eakinrespiratory.com\/therapies\/high-flow-oxygen-therapy\/\">HFOT<\/a>) y la presi\u00f3n positiva continua en las v\u00edas respiratorias (<a href=\"https:\/\/www.eakinrespiratory.com\/therapies\/cpap\/\">CPAP<\/a>) para oxigenar adecuadamente a los pacientes antes y despu\u00e9s de la operaci\u00f3n, con el potencial de mejorar el flujo de pacientes en los quir\u00f3fanos y reducir los ingresos postoperatorios en la UCI.\u00a0<\/p><p>El uso de la oxigenoterapia de alto flujo (HFOT) est\u00e1 aumentando tanto para la preoxigenaci\u00f3n como para la ventilaci\u00f3n apneica (especialmente en casos de ORL), pero tambi\u00e9n para el tratamiento de v\u00edas a\u00e9reas dif\u00edciles y la intubaci\u00f3n de secuencia r\u00e1pida (RSI). Existen varios dispositivos para el suministro de HFOT. Sin embargo, los dispositivos que solo suministran HFOT presentan limitaciones. A menudo, estos pacientes pueden permanecer m\u00e1s tiempo en recuperaci\u00f3n, pero la posibilidad de administrar CPAP r\u00e1pidamente puede acortar su tiempo de recuperaci\u00f3n, lo que contribuye al flujo de los pacientes en el quir\u00f3fano.\u00a0<\/p><p>En el Reino Unido, una de cada tres personas que acceden a los servicios sanitarios padece dos o m\u00e1s enfermedades cr\u00f3nicas, y uno de cada tres adultos ingresados de urgencia en un hospital brit\u00e1nico presenta cinco o m\u00e1s comorbilidades [<a href=\"https:\/\/bestpractice.bmj.com\/info\/the-impact-of-comorbidities-on-health-services\/\" target=\"_blank\" rel=\"noopener\">3<\/a>]. Las comorbilidades son m\u00e1s frecuentes en la poblaci\u00f3n obesa, incluidas las que requieren intervenci\u00f3n quir\u00fargica. El Informe Regional de Obesidad 2022 de la Organizaci\u00f3n Mundial de la Salud estim\u00f3 que el 26 % de la poblaci\u00f3n en Irlanda y el 28 % en el Reino Unido [<a href=\"https:\/\/apps.who.int\/iris\/handle\/10665\/353747\" target=\"_blank\" rel=\"noopener\">5<\/a>]\u00a0son obesos, con un IMC superior a 30. En el postoperatorio, se extub\u00f3 al paciente y se le administr\u00f3 CPAP mediante mascarilla facial con un 60 % de ox\u00edgeno, que se redujo durante la hora siguiente. El paciente fue trasladado a la UCI para observaci\u00f3n durante la noche, donde continu\u00f3 mejorando y fue considerado estable para ser dado de alta esa misma noche.\u00a0\u00a0<\/p><p>El paciente ten\u00eda antecedentes de apnea obstructiva del sue\u00f1o (AOS), una afecci\u00f3n frecuente entre los pacientes con obesidad grave, que se asocia a un mayor riesgo de episodios cardiopulmonares y a una tasa de mortalidad significativa. La recuperaci\u00f3n mejorada despu\u00e9s de la cirug\u00eda (ERAS), en sus directrices de 2021 para los cuidados perioperatorios en cirug\u00eda bari\u00e1trica, recomienda que los pacientes con AOS en tratamiento domiciliario con CPAP lo utilicen en el postoperatorio inmediato y que los pacientes con s\u00edndrome de hipoventilaci\u00f3n por obesidad (SHO) presentan un mayor riesgo de acontecimientos respiratorios adversos [<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00268-021-06394-9#citeas\" target=\"_blank\" rel=\"noopener\">2<\/a>].\u00a0<\/p><p>La VMNI postoperatoria debe considerarse liberalmente durante el postoperatorio inmediato, sobre todo en presencia de hipoxemia. El uso de tratamiento con CPAP\/BiPAP, cuando es necesario, puede disminuir el riesgo de complicaciones cardiopulmonares en pacientes con SAO sometidos a cirug\u00eda [<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00268-021-06394-9#citeas\" target=\"_blank\" rel=\"noopener\">2<\/a>]\u00a0<\/p><p>Aunque este paciente fue trasladado a la UCI para recibir cuidados posteriores, tal fue su mejor\u00eda que fue dado de alta sin necesidad de una costosa pernoctaci\u00f3n y se garantiz\u00f3 la disponibilidad de una cama en la UCI para el siguiente paciente.
