En todo el pa\u00eds existen disparidades en la gesti\u00f3n cl\u00ednica y los resultados.<\/p><p>Las modalidades de asistencia respiratoria como la oxigenoterapia de alto flujo (<a href=\"https:\/\/eakinrespiratory.com\/therapies\/high-flow-oxygen-therapy\/\">OAF<\/a>) y la presi\u00f3n positiva continua en las v\u00edas respiratorias (<a href=\"https:\/\/eakinrespiratory.com\/therapies\/cpap\/\">CPAP<\/a>) han demostrado mejorar los resultados de los pacientes en el tratamiento de la neumon\u00eda. Los estudios han demostrado que estas modalidades mejoran la oxigenaci\u00f3n, reducen la dificultad respiratoria y evitan potencialmente la necesidad de ventilaci\u00f3n mec\u00e1nica invasiva, reduciendo as\u00ed las complicaciones y acortando las estancias hospitalarias (Crimi et al., 2020; Frat et al., 2015). Sin embargo, estas estrategias tambi\u00e9n tienen implicaciones para la gesti\u00f3n de la HAP, aunque con algunas diferencias de enfoque debido a la naturaleza distinta de la HAP.<\/p><p>Ante todo, todos los pacientes con neumon\u00eda, ya sea adquirida en la comunidad o en el hospital, deben recibir una oxigenoterapia adecuada, a la vez que se supervisa la saturaci\u00f3n de ox\u00edgeno y la concentraci\u00f3n de ox\u00edgeno inspirado. El objetivo sigue siendo el mismo: mantener la tensi\u00f3n arterial de ox\u00edgeno (PaO2) igual o por encima de 8 kPa y la saturaci\u00f3n de ox\u00edgeno (SpO2) entre el 94 % y el 98 %. Este m\u00e9todo garantiza una oxigenaci\u00f3n adecuada tanto en los pacientes con NAC como con HAP, aunque esta \u00faltima puede tener complicaciones, como una mayor resistencia a los antibi\u00f3ticos y comorbilidades.<\/p><p>Adem\u00e1s, las t\u00e9cnicas de desobstrucci\u00f3n de las v\u00edas respiratorias deben tenerse en cuenta en todos los pacientes con neumon\u00eda, en particular en los que presentan acumulaci\u00f3n de esputo y dificultad para expectorar, independientemente de que la neumon\u00eda sea adquirida en la comunidad o en el hospital. Esto resulta a\u00fan m\u00e1s pertinente en los casos de HAP, en los que los pacientes pueden tener afecciones pulmonares preexistentes o sistemas inmunitarios debilitados, lo que aumenta el riesgo de complicaciones respiratorias.<\/p><p>Para casos m\u00e1s graves de neumon\u00eda, la ventilaci\u00f3n no invasiva y la presi\u00f3n positiva continua en las v\u00edas respiratorias suelen no recomendarse como tratamientos prioritarios en caso de insuficiencia respiratoria causada por NAC. No obstante, la neumon\u00eda adquirida en el hospital puede ser objeto de consideraciones similares, aunque la decisi\u00f3n de iniciar un tratamiento no invasivo debe tomarse con cautela y tras consultar a los especialistas en cuidados intensivos. Tanto en la NAC como en la HAP, si se estima necesario un tratamiento de soporte no invasivo, \u00e9ste s\u00f3lo debe realizarse en un entorno de cuidados intensivos dotado de acceso inmediato a personal experto que permita una transici\u00f3n r\u00e1pida a la ventilaci\u00f3n invasiva si fuera necesario.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6659ee1 elementor-widget elementor-widget-spacer\" data-id=\"6659ee1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fe1d096 elementor-widget elementor-widget-heading\" data-id=\"fe1d096\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Oxigenoterapia de alto flujo\n<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4757dbb elementor-widget elementor-widget-spacer\" data-id=\"4757dbb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b4318e1 elementor-widget elementor-widget-text-editor\" data-id=\"b4318e1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Se han publicado numerosos estudios en los que se compara la eficacia de la oxigenoterapia convencional con la OAF y, desde la pandemia de COVID-19, se observa un aumento del uso de la OAF a escala mundial. El tratamiento con OAF en pacientes con insuficiencia respiratoria hipox\u00e9mica de intensidad moderada a grave es un m\u00e9todo prometedor para mejorar los niveles de ox\u00edgeno y reducir la necesidad de intubaci\u00f3n y asistencia respiratoria intensificada, en comparaci\u00f3n con la oxigenoterapia convencional (Crimi et al., 20).<\/p><p>Por lo general, la OAF se utiliza en pacientes que respiran por s\u00ed mismos y necesitan ox\u00edgeno a flujos m\u00e1s elevados. Este tratamiento, que se administra a trav\u00e9s de una c\u00e1nula nasal, suministra gases calentados y humidificados a velocidades de hasta 80 litros por minuto y concentraciones de ox\u00edgeno que oscilan entre el 21 % y el 100 %. Cabe destacar que los estudios han demostrado que el OAF no solo mejora los niveles de comodidad, sino que tambi\u00e9n alivia la gravedad y reduce la frecuencia respiratoria (Frat et al, 2015). Adem\u00e1s, el OAF ayuda a la depuraci\u00f3n mucociliar, disminuye la demanda respiratoria, aumenta la oxigenaci\u00f3n y aumenta la ventilaci\u00f3n alveolar al reducir el espacio muerto anat\u00f3mico. Gracias a la reducci\u00f3n del esfuerzo respiratorio y el tratamiento de la oxigenaci\u00f3n, la OAF podr\u00eda evitar el ingreso en la UCI y la ventilaci\u00f3n mec\u00e1nica invasiva.<\/p><p>Una de las ventajas significativas de la OAF en el tratamiento de la neumon\u00eda es su capacidad para administrar concentraciones de FiO2 m\u00e1s altas de forma eficaz, mejorando as\u00ed la oxigenaci\u00f3n y aliviando la hipoxemia. Esto resulta decisivo, pues la hipoxemia es una complicaci\u00f3n frecuente de la neumon\u00eda y una de las principales causas de mortalidad. La OAF puede proporcionar niveles de FiO2 que oscilan entre 0,21 y 1,0, lo que permite una regulaci\u00f3n precisa que satisfaga las necesidades de cada paciente sin las complicaciones asociadas a las mascarillas tradicionales o a los procedimientos invasivos.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-055123e elementor-widget elementor-widget-spacer\" data-id=\"055123e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0b344f7 elementor-widget elementor-widget-heading\" data-id=\"0b344f7\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Pasar a CPAP\n<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c8583d8 elementor-widget elementor-widget-spacer\" data-id=\"c8583d8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-37ade7e elementor-widget elementor-widget-text-editor\" data-id=\"37ade7e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Otra opci\u00f3n es pasar al tratamiento con CPAP si la saturaci\u00f3n de ox\u00edgeno del paciente no mejora con la OAF. La CPAP, a diferencia de la OAF, administra un nivel fijo de presi\u00f3n positiva mientras el paciente inspira y espira. No act\u00faa como ventilador, sino que ayuda a ferulizar las v\u00edas respiratorias para mantener un patr\u00f3n respiratorio natural. Ahora bien, es fundamental una cuidadosa supervisi\u00f3n, valoraci\u00f3n y selecci\u00f3n de los pacientes para obtener unos resultados \u00f3ptimos y minimizar las complicaciones.
