Sin embargo, de forma aislada, estas cifras no pueden mostrar una imagen completa y solo pueden considerarse como un resumen actual, en lugar de un \u00edndice de predictibilidad.<\/p><p>El hecho de que exista un \u00edndice \u00fanico y f\u00e1cil de calcular que tenga en cuenta todos estos factores combinados podr\u00eda ayudar al m\u00e9dico a organizar a los pacientes en lugar de esperar a llegar a una fase en la que simplemente sea demasiado tarde para intubar o intensificar el tratamiento.\u00a0 El \u00edndice de ROX se dise\u00f1\u00f3 para ayudar a determinar si es seguro intubar al paciente o no, ya que esperar demasiado tiempo para intubar a un paciente tiene sus inconvenientes. En este estudio, llegaron a la conclusi\u00f3n de que, a las 12 horas, un \u00edndice de ROX de \u2265 4,88 significaba que el paciente no necesitaba ser intubado.<\/p><p>Esto se calcul\u00f3 en un conjunto de pacientes que recibieron un m\u00ednimo de 30\u00a0L de flujo y 100\u00a0% de FiO<sub>2<\/sub>. El objetivo era conseguir saturaciones de O<sub>2<\/sub>\u00a0superiores al 92\u00a0%. El estudio tambi\u00e9n demostr\u00f3 que la duraci\u00f3n mediana de la OAF bien tolerada fue de tres d\u00edas, mientras que, en el caso de los pacientes que no la toleraron bien, fue de aproximadamente un d\u00eda.<\/p><p>Despu\u00e9s de que Roca et al. llevaran a cabo este estudio, volvieron a analizar el \u00edndice de ROX y realizaron en 2019 un estudio de cohorte observacional prospectivo durante 2 a\u00f1os esta vez [<a href=\"https:\/\/www.atsjournals.org\/doi\/pdf\/10.1164\/rccm.201803-0589OC\" target=\"_blank\" rel=\"noopener\">6<\/a>]. Los autores concluyeron que un ROX menor de 2,85, menor de 3,47 y menor de 3,85 a las 2, 6 y 12 horas del inicio de la OAF, respectivamente, eran predictores del fracaso de la OAF y de que el \u00edndice de ROX era mejor que observar la SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>, la FR, la PaCO<sub>2<\/sub>, el flujo, la SpO<sub>2<\/sub>, la FiO<sub>2<\/sub>\u00a0y el lactato para predecir la necesidad de ventilaci\u00f3n mec\u00e1nica.
