<p>Nur selten gab es so fantasievolle Ideen, solchen Enthusiasmus und solche Ablehnung, und so viele unwissenschaftliche Beobachtungen und Studien zu nur einem klinischen Bild. Zwar gibt es unter diesen Aussagen herausragende Ausnahmen, doch die geringe Qualit\u00e4t und der Mangel an pr\u00e4zisen Daten bei den meisten Ver\u00f6ffentlichungen zur Reanimation von Neugeborenen ist bemerkenswert.<sup>[<a href=\"https:\/\/journals.lww.com\/anesthesia-analgesia\/citation\/1953\/07000\/A_Proposal_for_a_New_Method_of_Evaluation_of_the.6.aspx\" target=\"_blank\" rel=\"noopener\">1<\/a>]<\/sup><\/p><p>Virginia Apgar, die dies 1953 schrieb, wurde wahrscheinlich in diesem Gef\u00fchl best\u00e4tigt.<\/p> <p>In den 1950er Jahren bef\u00fcrworteten manche noch, dass Neugeborenen, die eine Intervention bei der Geburt ben\u00f6tigten, der Magen mit Sauerstoff aufgepumpt werden sollte<sup>[<a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/j.1651-2227.1963.tb03777.x\" target=\"_blank\" rel=\"noopener\">3<\/a>]<\/sup>. Apgar ver\u00f6ffentlichte 1963 tats\u00e4chlich einen Artikel, der zeigte, dass sich der Sauerstoffgehalt im Blut bei einer Bel\u00fcftung der Lunge verbesserte<sup>[<a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/j.1651-2227.1963.tb03777.x\" target=\"_blank\" rel=\"noopener\">3<\/a>]<\/sup>.<\/p><p><\/p><p>Und wie sieht es heute aus? Die Stabilisierung bei der Geburt ist in den vergangenen Jahren zwar st\u00e4rker in den Fokus der Forschung ger\u00fcckt, doch viele Aspekte der Versorgung im Krei\u00dfsaal sind immer noch zu wenig erforscht.<\/p> <p>Eine in zahlreichen Studien untersuchte Intervention ist das optimale Nabelschnurmanagement, und inzwischen wurde nachgewiesen, dass sowohl reife Neugeborene als auch lebhafte Fr\u00fchgeborene nach der Geburt von einem um einen gewissen Zeitraum verz\u00f6gerten Abklemmen der Nabelschnur profitieren.<sup>[<a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(23)02468-6\/abstract\" target=\"_blank\" rel=\"noopener\">4<\/a>]<\/sup>.<\/p> <p>Dies hat das Interesse an Atemunterst\u00fctzung bei noch nicht abgenabelten Neugeborenen und die Hoffnung, dass mehr Neugeborene von diesen Vorteilen profitieren k\u00f6nnen, sowie die Hoffnung auf eine m\u00f6gliche Verbesserung des \u00dcbergangs geweckt.<\/p> <p>Diese Intervention wurde k\u00fcrzlich in zwei gro\u00dfen randomisierten, kontrollierten Studien untersucht. Keine davon lieferte \u00fcberzeugende Evidenz f\u00fcr einen Vorteil dieser inzwischen h\u00e4ufig angewandten Praxis (Metaanalysen k\u00f6nnten allerdings weitere Informationen liefern).<\/p><p>In der VentFirst-Studie<sup>[<a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2818953\" target=\"_blank\" rel=\"noopener\">5<\/a>]<\/sup> wurden Neugeborene, die nach weniger als 29 Schwangerschaftswochen geboren wurden, randomisiert: entweder auf um 30 bis 60\u00a0Sekunden verz\u00f6gerte Abnabelung gefolgt von Standard-Reanimation, oder 120\u00a0Sekunden Atemunterst\u00fctzung (CPAP bei guter Atmung, oder \u00dcberdruckbeatmung bei schlechter Atmung), gefolgt vom Abklemmen der Nabelschnur.<\/p> <p>Bei der Rate von IVH oder Tod bestanden keine signifikanten Unterschiede zwischen den beiden Gruppen. In der ABC3-Studie<sup>[<a href=\"https:\/\/www.thelancet.com\/journals\/lanepe\/article\/PIIS2666-7762(24)00315-6\/fulltext\" target=\"_blank\" rel=\"noopener\">6<\/a>]<\/sup> wurden Neugeborene, die nach weniger als 30\u00a0Schwangerschaftswochen geboren wurden, randomisiert: entweder auf zeitbasiertes Abklemmen der Nabelschnur (idealerweise nach 30 bis 60\u00a0Sekunden), oder auf physiologiebasiertes Abklemmen, wobei Atemunterst\u00fctzung \u00fcber eine Gesichtsmaske bereitgestellt und die Nabelschnur nach Erreichen der \u201ephysiologischen Stabilit\u00e4t\u201c abgeklemmt wurde. Physiologische Stabilit\u00e4t ist als eine Herzfrequenz von mehr als 100\u00a0bpm und S\u00e4ttigungen \u00fcber 85\u00a0% definiert.<\/p> <p>Bei den Neugeborenen in dieser Gruppe wurde das Abklemmen der Nabelschnur um 3 bis 10\u00a0Minuten verz\u00f6gert.<\/p> <p>Beim prim\u00e4ren Ergebnis des \u201eintakten \u00dcberlebens nach der Entlassung aus der Neugeborenen-Intensivstation\u201c gab es keinen signifikanten Unterschied zwischen den beiden Gruppen. Allerdings wies eine Untergruppenanalyse auf einen potenziellen Vorteil f\u00fcr m\u00e4nnliche Neugeborene hin.<\/p> <p>Es wurde beobachtet, dass Neugeborene aus der Gruppe mit physiologiebasiertem Abklemmen der Nabelschnur bei der Aufnahme auf die Neugeborenenstation mit einer h\u00f6heren Wahrscheinlichkeit hypotherm waren. Hypothermie bei der Aufnahme ist bekannterma\u00dfen ein bedeutsamer Risikofaktor f\u00fcr die Mortalit\u00e4t unter Neugeborenen<sup>[<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0022347616300646\" target=\"_blank\" rel=\"noopener\">7<\/a>]<\/sup>. Daher k\u00f6nnte es dieser Faktor sein, der die in fr\u00fcheren Arbeiten gezeigten potenziellen Vorteile einer l\u00e4ngeren Nabelschnurtransfusion vor der Abklemmung reduziert.<\/p><p><\/p><p>Weiterhin wurde gezeigt, dass die Anwendung einer Gesichtsmaske bei der Geburt zu Apnoe bei Neugeborenen f\u00fchren kann, die zuvor geatmet haben<sup>[<a href=\"https:\/\/fn.bmj.com\/content\/108\/6\/594.abstract\" target=\"_blank\" rel=\"noopener\">8<\/a>]<\/sup>. M\u00f6glicherweise darf ein Ansatz mit Atemunterst\u00fctzung w\u00e4hrend der verz\u00f6gerten Nabelschnurabklemmung nicht der generelle Ansatz f\u00fcr alle Neugeborenen sein und sollte denjenigen vorbehalten bleiben, die nicht lebhaft sind. Allerdings muss dieser Ansatz wiederum in gut gestalteten Studien evaluiert werden.<\/p><p><\/p><p>Eine Grundfrage der Stabilisierung im Krei\u00dfsaal ist, wie der Atem\u00fcbergang bei der Geburt bestm\u00f6glich unterst\u00fctzt werden kann. Dahinter steht die Tatsache, dass die beiden gro\u00dfen Leitlinien f\u00fcr die Stabilisierung von Neugeborenen bei der Geburt \u2013 Newborn Life Support (NLS) und das Neonatal Resuscitation Program (NRP) \u2013 unterschiedliche Empfehlungen f\u00fcr Neugeborene geben, die bei der Geburt schlecht atmen.<\/p> <p>Die NLS-Leitlinien empfehlen, mit f\u00fcnf \u201eInflationsatemz\u00fcgen\u201c zu beginnen, wobei die verl\u00e4ngerten Inflationen 2 bis 3\u00a0Sekunden dauern, gefolgt von \u201eBel\u00fcftungsatemz\u00fcgen\u201c, die jeweils eine Sekunde dauern und mit einer Frequenz von 30 pro Minute gegeben werden. Das NRP empfiehlt dagegen eine \u00dcberdruckbeatmung mit einer Frequenz von 40 bis 60 pro Minute.<\/p> <p>Das Monitoring von Interventionen bei der Geburt hat gezeigt, dass selbst beim Versuch, Inflationsatemz\u00fcge von 2 bis 3\u00a0Sekunden zu verabreichen, eine gro\u00dfe Spanne bei der tats\u00e4chlichen Dauer der verabreichten Atemz\u00fcge besteht<sup>[<a href=\"https:\/\/fn.bmj.com\/content\/97\/4\/F249.short\" target=\"_blank\" rel=\"noopener\">9<\/a>]<\/sup>, und dass es bei l\u00e4ngerer Inflationsdauer zu einer fr\u00fcheren Stimulation der Spontanatmung kommen kann<sup>[<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0378378216300445\" target=\"_blank\" rel=\"noopener\">10<\/a>]<\/sup>.<\/p> <p>Aber wie lange ist zu lange? Tier- und Pilotstudien weisen darauf hin, dass \u201eanhaltende Inflationen\u201c von 10 bis 20\u00a0Sekunden vorteilhaft sein k\u00f6nnten.
